Rib Pain and Costovertebral Joint Dysfunction: The Upper Back Pain Connected to Your Ribs
July 21, 2026
Of all the upper back pain complaints Dr. Jeff Hall evaluates, one of the most misunderstood involves the rib pain and the condition called Costovertebral Joint Dysfunction. Patients often arrive convinced something is wrong with their heart or lungs — chest tightness, sharp pain with breathing, a feeling like they can’t get a full breath — only to discover the actual source is a joint most people don’t even know exists: the costovertebral joint, where each rib attaches to the spine.
What Are Costovertebral Joints?
Each of your 12 pairs of ribs connects to your thoracic spine through small joints called costovertebral joints. These joints allow the subtle motion your rib cage needs to expand and contract with every breath. When a costovertebral joint becomes restricted, irritated, or slightly misaligned — often called a rib subluxation — the result can be surprisingly intense localized pain, muscle spasm along the rib, and a sensation that mimics chest or breathing problems.
Common triggers include:
- A sudden twisting movement, such as reaching awkwardly or lifting while rotated
- A forceful cough or sneeze, which can jar the rib joint out of its normal position
- Prolonged poor posture, placing chronic asymmetric stress on the joints
- Direct trauma, including sports collisions or minor falls
One CNS Orthopedics patient described suffering from what felt like terrible chest pain, only to learn that correcting her ribs and diaphragm alignment resolved both the pain and her breathing difficulty entirely — a pattern that plays out more often than most patients expect.
Why This Gets Misdiagnosed So Often
Because rib and costovertebral pain can genuinely mimic cardiac or pulmonary symptoms, it’s important to rule out serious causes first — and we always encourage patients to seek emergency care if there’s any doubt about chest pain. But once cardiac and pulmonary causes have been ruled out, a large percentage of ongoing “chest wall” or “between the shoulder blades” pain traces back to mechanical rib dysfunction that responds extremely well to hands-on treatment — something general upper back pain advice doesn’t always address directly.
How CNS Orthopedics Treats Rib and Costovertebral Pain
Chiropractic Adjustments — Targeted adjustments to the thoracic spine and rib joints restore normal motion, often providing rapid relief for rib-related pain.
Low Level Laser Therapy — Reduces inflammation in the irritated joint and surrounding intercostal muscles.
Clinical Exercise — Breathing mechanics and postural exercises help prevent the rib dysfunction from recurring, especially for patients whose pain is tied to posture or repetitive movement patterns.
Diagnostic Testing — A thorough exam distinguishes costovertebral dysfunction from other causes of upper back and chest wall pain, ensuring you’re treated for what’s actually happening.
Chest and Upper Back Pain Isn’t Always What It Seems
If you’ve had cardiac and pulmonary causes ruled out and you’re still dealing with sharp, breath-limiting upper back or chest wall pain, a rib or costovertebral issue may be the answer you haven’t considered yet.
