WHAT TO EXPECT WHEN YOU COME FOR AN APPOINTMENT
Printable New Patient Forms
As with any doctor’s visit, you’ll start by finishing some new patient paperwork. Get a head start on that by downloading and pre-completing the documents available here. Simply print them at home and complete the form before your first office visit. Bring your completed copy to the office at the time of your appointment. This will save you time and make the office administration process faster and easier.
Upon meeting with the doctor, you’ll discuss your concerns and expectations as well as have the chance to ask any questions you may have. We will perform an Ortho-Neuro examination, X-Rays, Ultrasound, and Blood test. Sometimes a MRI or NCV test may be needed. Our goal is to get your low back pain and/or other joint pain under control as soon as possible.
