Cervicogenic Headaches: When Neck Problems Are the Real Source of Your Headaches

Most people assume a headache starts in the head. But for a significant number of chronic headache sufferers, the actual source is several inches lower — in the joints, muscles, and nerves of the upper neck. These are called cervicogenic headaches, and they’re one of the most frequently mistreated headache types, because patients spend years being managed for migraines or tension headaches that medication never quite resolves. At Chattanooga Non-Surgical Orthopedics, Dr. Jeff Hall regularly identifies neck dysfunction as the true root cause behind headaches patients had almost stopped expecting to fix.

How a Neck Problem Becomes a Headache

The uppermost joints of your cervical spine share nerve pathways with structures in your head and face, a phenomenon called “referred pain.” When the joints, discs, or muscles of the upper neck become irritated, misaligned, or inflamed, that irritation can be perceived by the brain as pain in the head — even though the actual dysfunction is happening in the neck.

Common contributors include:

  • Cervical joint dysfunction or misalignment, particularly in the upper cervical spine near the base of the skull
  • Forward head posture, common with prolonged desk and phone use, which places chronic strain on the upper neck muscles and joints
  • Whiplash or prior neck injury, which can leave lasting joint irritation long after the initial injury seems to have healed
  • Tight suboccipital muscles at the base of the skull, which can directly compress sensitive nerve structures

How to Tell the Difference From a Migraine

Cervicogenic headaches have a recognizable pattern that sets them apart from migraines and typical tension headaches. They usually start as a dull ache at the base of the skull or in the neck and spread forward toward the forehead or behind one eye — almost always on one side. They’re frequently triggered or worsened by specific neck movements or sustained postures (like looking down at a phone), and patients often notice genuinely restricted neck range of motion alongside the headache itself, something migraines typically don’t involve.

Because the symptoms can overlap, cervicogenic headaches are commonly misdiagnosed and treated as migraines with medication that manages the pain temporarily but never addresses the neck dysfunction actually driving it.

How CNS Orthopedics Treats Cervicogenic Headaches

Chiropractic Adjustments — Precise adjustments to the upper cervical spine are often the most direct way to resolve the joint dysfunction generating referred head pain.

Low Level Laser Therapy — Reduces inflammation in irritated cervical joints and the tight suboccipital muscles that frequently accompany cervicogenic headaches.

Clinical Exercise — Deep neck flexor strengthening and postural correction address the forward-head-posture pattern that’s often at the root of recurring headaches — particularly important for anyone working a desk job.

Non-Surgical Spinal Decompression — For headaches linked to cervical disc involvement, decompression can relieve the underlying structural pressure.

One CNS Orthopedics patient regained full neck range of motion and feeling in her fingers through a combination of decompression, exercise, and PRP therapy — improvements she hadn’t realized she’d lost until they returned, and the kind of whole-neck correction that often resolves headache patterns as a byproduct.

If Your Headaches Won’t Respond to Medication, Check Your Neck

Chronic headaches that haven’t responded well to typical migraine or tension-headache treatment are worth evaluating from a completely different angle — one that starts at the neck instead of the head.
